True safety improvement requires continuous learning, strong leadership commitment, and consistent application of lessons learned—so that the same mistakes are not repeated. It was with this in mind that the Rig Site Safety Working Group came together in March to consider repeat incidents – with a particular focus on Camlocks, Extension Bars and Baskets at their March DCIIRP ‘History Repeats’.
Following a welcome from Kyle Koziol (Ventia) and a Safety Moment – ‘Return to Learn’ from Ben Corbett (Origin Energy), the Working Group took some time to consider repeat incidents:
- Hayley Forbes (Coho Resources) with a review of Camlock Fitting Incidents.
- Les Irwin & Mitchell Finch (Ventia) with a review of Storage Basket Incidents.
- Jason Burns (Wild Desert) with a review of Hand Tool Incidents (Stilson’s, Chain Tongs & Wrenches).
- David Machin & Richard Darby (Ensign) with a review of Flogging Spanner Incidents.
The Forum topic sparked a lot of interest within our D&C community – great engagement, feedback and discussions followed each presentation.
Following these incident reviews, Mike O’Connell (Santos) then presented a Safety Improvement Plan Overview (Camlocks, Extension Bars & Baskets) – proposing the Working Group Look back on previous incidents across operators and contractors and review current equipment and operational practices with the frontline workers with the objective to identify what further industry wide improvements could be considered to minimise occurrence of such incidents in the future.

Mark Tonin (Wild Desert) next presented the relaunch of the Short Service Employee (SSE) resources, including:
- An updated SSE Editable Matrix
- An updated SSE Employee Management Form
- Updated SSE Implementation Materials.
