Incident Review
IP was conducting the monthly routine cleaning of high suction strainer. The top lid was to be put back on top of the sea suction at the end of the day. The lid is weighing approximately 100 kilos, and a chain hoist was used to lift it to the position on top. The hoist was secured to the celling in a pad with a rope. To get the lid to move free from the sea suction while lifting, IP was pulling it away from the unit. When the lid was approx. 30cm from the deck, the securing of the chain hoist in the celling broke and the lid fell on his leg. The lid hit IP’s leg. The other engine crew helped remove the lid and they put the lid back on the sea suction.
