For our final event of 2025, our Qld. Process Safety Working Group held their Process Safety Forum in Brisbane, with a focus on Preventing Major Incidents and Bowties.
Thank you to Allyson Woodford (GM Engineering & Technical Solutions, APA Group) for her thoughtful introduction and event welcome, before presenting ‘The Things That Matter’, a reminder to all delegates that we must not forget events of the past, and why we must rigorously continue our focus on incident prevention.

The ensuing presentations followed:
- Dave Smith (Principal Safety Engineer, Shell QGC) with his presentation ‘Process Safety Key Performance Indicator Insights 2025’ and an incident review.
- Sheila Wujanto-Hawe (Process Safety Engineer, Jemena) with her presentation ‘Guidance for Operationalising Bowties’.
- Pranesh Khatri (Process Safety Manager, Santos) with his presentation ‘Santos: Development and Operationalisation of Bowties’.
- Dale Whiting (Process Safety Engineering Lead, ConocoPhillips) and Zeeshan Qureshi (Principal HSE Risk and Process Safety, Arrow Energy) presented on the Process Safety Working Group’s Operating Integrity Project.
- Shoena Messner (RSHQ) presented ‘Operationalising Operating Integrity - A Regulator View’.
Following a panel session, Richard Pocock (Worley) closed the event, thanking delegates for their attendance and engagement.
