An LEB & Toolbox related to Operating a Vessel - Person Fall From Height.
What Happened?
When conducting routine daily inspections within the engine room of an offshore vessel, a worker fell through an open 900mm x 500mm penetration in a grid mesh floor. They caught their arm on the adjacent sink and ladder as they fell, preventing a 2.5 metre fall to the deck below. The worker managed to climb out of the penetration via the ladder. They sustained injuries to their pectoral / brachial plexus during the fall. A fall from this height has the potential to cause a fatality or critical life-threatening injuries.
