Loading/Unloading - Person in Line of Fire

A Piling Crane was transported on a flatbed trailer with the crane block secured through crane skirt in single wrap style using a transport chain.

As part of the crane setup, whilst booming up, the travel chain failed and a piece of chain link struck the spotter below the knee who was standing to the side of the crane. The crane block (which was secured by the chain) swung in a pendulum motion striking piling pipes stacked at grade. No person was in the line of fire.
