Drilling/Completing a Well-Person in Line of Fire

This flyer is the basis for a Learning Event related to person being in line of fire. 

A rig crew was preparing to remove bushings on the rig floor. During the operation of the winch line it suddenly released. The chain bushing pullers fell to the rig floor and hit the V-door gate.
