Our industry conducts thorough incident investigations and internal reviews. How do we share lessons learnt from these investigations across all rigs?

Solution

Conduct a quarterly forum for presenting lessons learnt from incident investigations and information about safety innovations. Develop a common template and process for presenting the outcomes of incident investigations.

Result

The Drilling and Completions Industry Incident Review Panel (DCIIRP) is a quarterly forum for the entire Drilling and Completions sector of the Queensland Natural Gas Exploration and Production Industry. The first DCIIRP was conducted in May, 2015. It is an open forum with a no blame culture focused on sharing and learning. Safer Together members can access past DCIIRP presentations here.
