On 2 September, the WA/NT Process Safety Working Group (PSWG) hosted its third Process Safety Incident Review (PSIR) workshop since inception. In line with Safer Together’s New Ways of Working adopted in response to COVID-19, the PSIR was held in webinar format.

More than 250 Safer Together member company representatives joined the webinar.

PSWG Co-Champions, Ralph Ellis (Wood) and Sander Stegenga (Shell) were masters of ceremony.  

Presentations shared the learning from the following incidents and innovations: Offtake Hose Failure, Simplicity From Complexity in Relation to the Simplification of Safety Cases, Flare Systems, LSD (Large Scale Deformation) and Bad Vibration, which was followed by interactive questions and answers with the viewers.
